SEA 00 letter Ser 00/445 dated Jun 19, 1014 (Signed by VADM Hilarides) released SEA 00 letter Ser 00/C002 dated May 20, 2013 (signed by VADM McCoy) which is the report of the FINAL COMMAND INVESTIGATION INTO THE FIRE THAT OCCURRED ONBOARD USS MIAMI (SSN 755) AT THE PORTSMOUTH NAVAL SHIPYARD ON 23 MAY 2012. The report was originally classified CONFIDENTIAL - RESTRICTED DATA until redacted and FOIA released at the NAVSEA FOIA reading room. (https://www.navsea.navy.mil/Contact/FOIA/Reading-Room/)

The investigation determined that the root cause of the severity of the fire is the deliberate act of arson and how the associated loss of response time impacted the crew and firefighters. Because of the dedicated fire safety and prevention efforts and the corresponding reduction of the number and severity of shipboard fires in overhaul, the NAVY developed over-time a cultural blindspot to a shipboard fire scenario as seen in MIAMI.
