San Francisco (SSN 711) Grounding (2005) Investigation Report

This is the FOIA-released redacted U.S. Navy Investigation Report on the grounding of USS San Francisco (SSN 711) on Jan 8, 2005 when it struck an uncharted sea mount while transiting at flank speed and submerged at a depth of 525 feet. The findings showed that while the charts did not include the sea mount, they did include nearby hazards in the vicinity of the grounding. The grounding occurred approximately 360 nm south of Guam. There was 1 fatality due to severe head injury.
